[Pediatrics] Intussusception : 장중첩입니다..!!
Ileum telescopes into cecum
-> Pain, obstuction, edema
-> compression of blood vessels
-> bowel ischemia
-> rectal bleeding (currant jelly stools)
Pathology
Occurs when a segment of the intestine telescopes into an adjacent segment.
-leads to obstruction, inflammation, and edema
-can lead to life-threatening bowel perforation and peritonitis
-most common site: ileocecal valve.
-cause unknown in most paitents
Clinical features
-sudden episodes of abdominal pain
-pain free and normal between episodes.
-inconsolable crying
-bilious vomiting
-sausage-shaped mass palpated in the right mid or upper abdomen.
-stool's consistency and color change (red, jelly-like stools) :
-diagnosis: abdominal ultrasound
Nursing intervention
priority is to restore bowel to normal position and function quickly
-Nonsurgical reduction using hydrostatic or air enema
-analgesics
-IV fluids
-NPO
-Nasogastric decompression
-Surgery
Inappropriate answers
1. The likelihood of intussusception reoccurring is low after treatment - not high risk of recurrence.
2. stool culture collection is not indicated. b/c intussusception is not caused by bacterial infection.
3. hyperosmotic lavatives are not indicated for clients with bowel obstruction and inflammation. b/c of the risk for increased intestinal pressure and perforation.
